How do I make an appointment?

To make an appointment with our clinic, please call (352) 265-8408.

What to bring to your appointment

  • Your insurance card
  • Physician referral forms (if required by insurance)
  • A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Any recent x-rays or appropriate records you may have

Insurance and payment information

University of Florida Comprehensive Epilepsy Program is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.

If you have any questions regarding which insurance plans we accept or any patient billing concerns, please call us at the phone number below. Questions regarding your coverage and benefits should be directed to your employer or insurance company.

Where do I go for my appointment?

Your clinic appointment will be located at the Shands Medical Plaza at the University of Florida.

Directions: From I-75, take exit #384 (Archer Road) west (towards the University of Florida) until you reach Gale Lemerand Drive. Take a left at Gale Lemerand Drive and then make a right at the Shands Cancer Center. Follow the signs to the Medical Plaza parking garage.

Most of the other activities of the UFCEP take place in Shands Hospital at the University of Florida and the Gainesville Veterans Administration Medical Center. Shands Hospital is a 618-bed hospital located in Gainesville, Florida, which serves as a tertiary referral center for the southeastern United States. In addition to state-of-the-art medical, surgical, and imaging facilities, Shands houses an Epilepsy Monitoring Unit that includes four adult and two pediatric rooms, specifically designed for long-term monitoring of seizures in patients with epilepsy. In addition to two excellent hospitals, the UFCEP also benefits from the intellectual and physical resources of the University of Florida McKnight Brain Institute.
